Instructions for Use

1. Preparation: Before beginning, consider your organization's practices and capabilities objectively. The assessment is most valuable when responses reflect your current reality rather than aspirations.

2. Taking the Assessment: For each of the 20 statements, select the option that best represents your organization's current situation on a scale from "Strongly Disagree" (1) to "Strongly Agree" (5). Consider your organization's typical patterns over the past 1-2 years rather than isolated incidents.

3. Complete All Questions: Ensure you respond to all statements. The assessment requires complete information to generate accurate results. You'll be prompted if any questions remain unanswered.

4. Reviewing Your Results: After submitting, you'll receive:
